Fishing from the bottom of a cliff at night alone  (bad enough, not reccommended) when I decided to adjust the focus on my headlight unscrewed it too far and the glass and bulb fell off into the sea. Had to climb the cliff in pitch darkness but for the light of a lighthouse every few seconds.

I always keep a loo roll in the tackle box for such an occasion

Wrapped up tightly in a waterproof bag, for obvious reasons

Power casting the top half of your rod into a very lumpy sea and managing to snap your line in the process so the top half of the rod flies well beyond recoverability.

Went night fishing so I brought my lantern - BROUGHT the matches but no petrol in the lantern
Went night fishing so I brought the head-torch - It lasted about 20 minutes and I was plunged into darkness. TEST YOUR GEAR BEFORE YOU GO!
